How do you approach level/world building?

Excuse me if I sound naive, but I'm looking for the best way to create outdoor levels. Do any decent standalone programs exist, (or other engines that have nice level-editors which can export formats for use with Ogre?), or am I better off creating my meshes in Maya(or something else) and then manually constructing my large outdoor scenes with Ogre?

Which programs do all of you use to create your levels and worlds? How do you approach scripting and entities? I'm mostly interested in using the PSM (Paging Scene Manager), as I would like to conserve the most resources when constructing larger outdoor worlds.

My main goal is to develop some decent large-sized outdoor "realms", where most of the player interaction/fighting will take place. I'm looking for some options in accomplishing this, and I'd appreciate any advice that anyone could provide.
