When you build simulator/device binaries against a fat lib, does this mean the eventual compiled application package will be larger, or does the linker strip out unused stuff? Are there any other disadvantages of fat libs over having separate simulator/device libs?

It's more of an issue of convenience. The linker will strip out any unneeded symbols, that includes architectures.
